Signposting Service

The Signposting Service helps people to find non-clinical support and activities in the local community that can benefit their health and well being. The team offer individually tailored solutions for each person that is referred to them.

What do we do?

There are many reasons why someone might need a little extra support. For instance they might be feeling lonely or isolated, have mobility problems, caring responsibilities, feel low or anxious or have been bereaved.

Whatever the issue, the service can signpost people to all sorts of support, some traditional and some less traditional. This can include art groups, debt counselling, gardening clubs, luncheon clubs, housing, volunteering activities, dance groups, housing advice and shopping services.

The service also directs people to organisations and groups supporting specific health conditions.

This one stop service gives people information about community groups and services local to them that can help them to improve their health and well being.

How do I access this service?

People over 18 who are registered with a GP in the PO1-PO6 postcodes can be referred to the Signposting Service.

Referrals can be made at GP practices, by health care professionals or by people representing voluntary and community organisations.

What happens next?

Once you have been referred, a member of the Signposting Service team will get in touch with you to discuss your needs and goals.

They will then provide you with information on services that may benefit you.
